What telehealth is and how your visits will work

Telehealth means receiving medical care through live, secure video, telephone, and secure electronic messaging rather than in person. Wellesley Medical is a telehealth-only, consultation-only practice: all visits with Dr. Lampert take place by secure video (with telephone as a backup if video fails), at a scheduled time, using Zoom or Google Meet, secure video platforms that meet applicable privacy and security standards. You will receive a link before each visit. You may complete visits from any private location within Massachusetts or Rhode Island.

Where you must be located

Dr. Lampert is licensed in Massachusetts and Rhode Island. You must be physically located in Massachusetts or Rhode Island at the time of every visit. We will confirm your identity and physical location at the start of each visit. If you are outside these states, the visit must be rescheduled — this is a licensing requirement, not a practice preference.

Benefits of telehealth

Convenience and access to care without travel; shorter waits; the ability to involve family members you choose; and continuity of care with a single physician.

Risks and limitations — read this section carefully

• No hands-on physical examination. Dr. Lampert cannot palpate, listen to your heart and lungs, or physically examine you through a screen. Some conditions cannot be fully evaluated by video, and this can, in some circumstances, contribute to an incomplete or less accurate assessment than an in-person examination would provide.
• Technology can fail. Poor connections, dropped video, or equipment problems can interrupt a visit or degrade the information available to Dr. Lampert, which could result in an incomplete or inaccurate diagnosis. If technology fails during a visit, we will attempt to reconnect, complete the visit by phone, or reschedule at no charge.
• When telehealth is not enough, you will be told. If at any point Dr. Lampert determines that your condition requires an in-person examination, imaging, or specialty care, he will tell you directly and refer you to appropriate in-person care. You agree to follow through on such referrals.
• Security. We use encrypted, access-controlled systems, but no electronic system can be guaranteed absolutely secure.

Emergencies

Telehealth is never appropriate for emergencies. If you experience a medical emergency — chest pain, difficulty breathing, signs of stroke, thoughts of harming yourself, or any condition you believe is life-threatening — call 911 or go to the nearest emergency department immediately. Do not use the patient messaging system for urgent or emergent symptoms; it is not monitored continuously.

Follow-up and continuity

You will be told how to obtain follow-up care — virtual or in-person — after every visit, including in the event of a technology failure, an unexpected outcome, or an emergency. Wellesley Medical is not a primary care practice and does not replace your primary care physician; we strongly recommend you maintain one.

Recording

Visits are not recorded by the practice. You agree not to record visits without Dr. Lampert's express consent.

Your rights

• You may withdraw your consent to telehealth at any time, including during a visit, without penalty and without affecting your right to future care or referral.
• You may ask questions about telehealth, your care, or these terms at any time.
• Withholding or withdrawing this consent will not affect your access to alternatives; because this is a telehealth-only practice, the alternative is referral to an in-person provider, which we will assist with on request.
• You have the same rights to confidentiality of your information as you would in in-person care.

What this practice is — and is not

Wellesley Medical is a physician-directed, telehealth-only practice focused on metabolic weight management, hormone optimization, and FDA-approved peptide therapy. It is a consultation and prescribing practice: we evaluate, prescribe, monitor, and adjust. We do not dispense medications, perform procedures, or provide in-person examinations.

We are not a primary care practice, an urgent care, or an emergency service. You should maintain a primary care physician for preventive care, acute illness, and conditions outside our scope. With your authorization, we will share relevant records with your PCP.

Laboratory testing

Baseline and monitoring labs are drawn at LabCorp or Quest Diagnostics locations near you; your lab order will specify which network to use. The initial evaluation fee includes the baseline laboratory panel. You are responsible for completing lab draws as ordered. Treatment decisions and prescription renewals depend on current labs; prescriptions may be paused if required monitoring labs are not completed. This is a safety rule, not an administrative one.

Cash-pay practice — no insurance

Wellesley Medical does not participate in or bill any insurance plan, including Medicare and Medicaid. Fees are due at the time of service. On request we will provide an itemized receipt; we make no representation that any insurer will reimburse you. Medication costs at the pharmacy and any imaging or specialty referrals are separate from our fees and may or may not be covered by your insurance.

Fees

• Initial evaluation (includes baseline labs): $600, due at booking.
• Ongoing membership: $350 per month, offered after the evaluation and activated only by your explicit written opt-in (Form 7). The evaluation fee never converts automatically into a membership, and your card is never charged for membership without your separate written authorization.
• A Good Faith Estimate of expected charges is provided to every self-pay patient (Form 8).

Communication

Routine questions go through the secure patient messaging system and are answered during business hours, typically within 1–2 business days. The messaging system is not monitored continuously and is never appropriate for emergencies — for emergencies, call 911.

Controlled substances (applies if testosterone therapy is prescribed)

Testosterone is a Schedule III controlled substance. If prescribed: Dr. Lampert will be your only prescriber for this medication; you agree not to obtain testosterone or related androgens from any other source (including other clinics, online sellers, or non-prescription sources); prescriptions are not refilled early or replaced if lost; the state prescription monitoring program will be checked as required by law; and you agree never to share, sell, or give your medication to anyone else. Violation of these terms is grounds for discontinuing controlled-substance prescribing and may be grounds for discharge from the practice.

Honest disclosure

You agree to provide complete and accurate information about your health history, current medications and supplements (from all sources), substance use, and symptoms, and to promptly inform Dr. Lampert of changes, new medications from other providers, hospitalizations, pregnancies, or side effects. Safe prescribing in a telehealth model depends on the accuracy of what you report.

Ending the treatment relationship

Either you or the practice may end the relationship at any time. If the practice ends it, you will receive: written notice; a reasonable period (no less than 30 days) of continued access for urgent issues; bridge prescriptions for a transition period where clinically appropriate; assistance identifying alternative care; and transfer of your records to a new provider on request. Cancelling your membership does not by itself end the physician-patient relationship; the obligations above apply regardless of payment status.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our duties

We are required by law to maintain the privacy and security of your protected health information (PHI), to provide you this Notice of our legal duties and privacy practices, to notify you following a breach of unsecured PHI, and to follow the terms of the Notice currently in effect.

How we may use and disclose your health information without your authorization

• Treatment. To provide, coordinate, and manage your care — for example, sending prescriptions to your pharmacy, ordering labs from LabCorp or Quest Diagnostics, or sharing records with a specialist we refer you to.
• Payment. To bill and collect payment for services — for example, processing your card payment through our payment processor.
• Health care operations. To run the practice — quality review, compliance, administrative services performed by our management company under a business associate agreement, and secure technology vendors (scheduling, messaging, records) who are contractually bound to protect your information.
• As required by law, including public health reporting, health oversight activities, judicial and administrative proceedings, law enforcement purposes as permitted by law, and to avert a serious threat to health or safety.

Uses and disclosures requiring your written authorization

Most uses and disclosures of psychotherapy notes; marketing; any sale of PHI; and any purpose not described in this Notice require your written authorization, which you may revoke at any time in writing (except to the extent we have already acted on it). We do not sell your information. We do not use your information for third-party marketing.

Your rights

• Access: inspect and receive a copy of your records, including an electronic copy, generally within 30 days of request.
• Amendment: request amendment of your records; if we deny the request, you may submit a statement of disagreement.
• Accounting of disclosures: receive a list of certain disclosures we have made.
• Restrictions: request restrictions on uses/disclosures; we must agree to restrict disclosure to a health plan for services you paid for in full out of pocket (which, as a cash-pay practice, applies broadly to your care here).
• Confidential communications: request that we contact you by specific means or at a specific location.
• Paper copy: receive a paper copy of this Notice at any time.
• Breach notification: be notified if a breach compromises your unsecured PHI.

Complaints

If you believe your privacy rights have been violated, you may complain to us at the contact below and/or to the U.S. Department of Health and Human Services, Office for Civil Rights. You will never be penalized or retaliated against for filing a complaint.
